Job Description:
We are seeking a dedicated and experienced Locum Band 5/6 UCR Intermediate Care Physiotherapist to join our team in Willerby. The successful candidate will be responsible for providing high-quality community-based care to adult patients in their own homes. Experience with System 1 is advantageous.
What your responsibilities will be for this Physiotherapy role?
As a Band 5/6 Locum Physiotherapist the responsibilities of the role include delivering comprehensive assessments and interventions for adult patients within the community, managing a diverse caseload that encompasses a variety of clinical needs such as falls prevention, frailty, long-term conditions, and end-of-life care. You will be expected to utilise your wide range of community clinical skills, including equipment management, core treatments, and assessments, while effectively responding to and triaging patient needs. The role also requires the ability to work independently in patients' homes, ensuring high standards of care are maintained at all times.
Requirements:
- HCPC registration
- Right to work in the UK
- Proven community experience
- Access to a personal vehicle for work purposes
- Experience with System 1 (desirable)
- Strong skills in responding and triaging, with a broad range of clinical community skills
- Experience in managing a caseload including falls, frailty, long-term conditions, and end-of-life care
